Because training involves a living animal which reacts to variations in its environment, Training Unlimited makes no representations that the training administered will accomplish every desired goal. However, we will extend every effort to satisfy your needs. I ______________________ understand that attendance of a training session is not without risk to myself, members of my family or guests who may attend; or any pet. I hereby waive and release Training Unlimited and its employees from any and all liability of any nature. This includes loss, death, injury, escape and damage to other persons and property.
Other Terms and Conditions:
1) I agree to cooperate with all instructions to the fullest of my capability. This means working with the animal in accordance with instructors’ recommendations, knowing full well that not practicing or following through may result in the animals’ behavioral regression.
2) All fees will be paid as stated above, at the time of each session, unless otherwise arranged. Boarding fees as follows:
30% (non-refundable) when booking boarding = ___________________

3) I, the animal(s) owner agree to be responsible for all veterinary costs associated with my animal whether due to injury or illness while boarding and agree to let the closest veterinarian, Laurel Stone Vet Clinic, treat my animal, unless otherwise stated. 4) If I chose to have my animal treated by another veterinarian I agree to pay $1/mile for transport to that vet, plus an hourly fee of $40 to Training Unlimited.
5) I agree to furnish current vaccination records which include rabies, distemper, parvo, influenza, leptospirosis, corona virus, and bordetella vaccines. I also understand that Lymes vaccine is highly recommended. I further agree to maintain my pet on a regular schedule of flea and tick preventative such as Frontline and heartworm preventative such as Interceptor.
6) As my animal(s) will be exposed to other animals, I understand that there is always the risk of contagion in the form of parasites, fungus or other contagious diseases and injury from boistrous activity. The only means of preventing this possibility is to quarantine my animal from others during his/her visit.
7) _______ Checking this space confirms my intention to not allow my pet exposure to other animals; though this may not totally prevent any possible chance of contagion and will reduce the quality of his/her visit at SMPR as he/she won’t be allowed to socialize with other animals
8) I confirm that my animal(s) are not aggressive to other animals. If so, I will state this fact and agree to their being isolated from other animals during their visit to SMPR.
